How to Make a Referral for Sub-acute or Psychiatric Residential Treatment
A referral for sub-acute or residential treatment may be made by anyone at any time. However, certain items are necessary to properly review and process the referral:
- About The Psychiatric Residential Treatment Center (PDF)
- Referral Form for Methodist Residential Programs (PDF) (must be completed by a clinician or therapist)
- Arkansas Total Care Residential Prior Authorization Form for Initial Request (PDF) (If your client has Arkansas Total Care, please fill out this form and fax back to us at 501-421-6477 or email it to Residential-Admissions@MethodistFamily.org.)

Referrals for qualified residential treatment programs (formerly therapeutic group homes) must be initiated by the Arkansas Division of Children and Family Services (DHS). Please contact the child or adolescent’s family service worker for a referral to this program.
